Harsh,

  The proforma is created from the Simulation Studio. To make one, select "new" from the "file" menu and select "new component"


  Basically what you need to do is specify the Type number on the first tab and then the parameters, inputs, and outputs, on the third tab. Hopefully you have a list of what the required parameters, inputs, and outputs are.
